I confirm that the information provided in this form is accurate and complete.
I understand that submitting this form is a request for a trial class and does not guarantee placement in a specific class. Class placement is determined by AA Classical Ballet School based on the student’s age, experience, and skill level.
I understand that ballet and physical activity involve inherent risks of injury. I voluntarily accept these risks on behalf of myself or my child while participating in a trial class.
To the fullest extent permitted by law, I release and hold harmless AA Classical Ballet School, its Artistic Directors, instructors, staff, volunteers, and facility owners from liability for any injury, accident, illness, or loss of personal property that may occur during or as a result of participation in a trial class, except where caused by negligence or where liability cannot legally be excluded.
I confirm that I have disclosed any medical conditions, injuries, allergies, or other information that may affect participation in ballet activities.
I authorize AA Classical Ballet School to provide appropriate first aid and seek emergency medical treatment if necessary when a parent or emergency contact cannot be reached. I understand that any medical expenses incurred are my responsibility.
I grant permission for AA Classical Ballet School to photograph and/or record the trial class and to use photographs or videos for promotional, educational, website, social media, and marketing purposes. If I do not wish to provide this consent, I will notify the school in writing before the trial class.
I consent to receiving email and telephone communications from AA Classical Ballet School regarding my trial class request, programs, and registration information.
